The CCCA has contracted with CareVio to support CCCA Participants per LOB in the following areas:
- Transitional Care Management – Focused support to help patients after an emergency room visit or an inpatient stay at a hospital, skilled nursing, or inpatient rehab facility.
- Longitudinal Comprehensive Case Management – Focused case management support to patients, including high risk pediatrics, with chronic health conditions in need of resources and support.
- Complex Case Management – Focused in-person or virtual help with medical, mental health, and/or social needs to build life skills and connect patients with the resources right for them.
- Special Populations – High Risk Maternity Case Management – Focused support and guidance for a healthy pregnancy as well as the arrival of the new family member.
