Role of Quality
Goals: Support practices to strategically drive performance in value-based contracts and, most importantly, meet the needs and improve the overall health and wellness of the patients that you serve.
Objectives: Stewards for overall quality operations and continuously optimizing value-based contract performance and patient outcomes through ongoing caregiver education and data sharing with a focus on quality, cost, and utilization.
Performance: Ensure patient outreach and engagement programs and workflows are designed, implemented, and sustained in a manner that supports value-based and risk contracts.

- Cancer Screening: Breast, Cervical and Colorectal
- Diabetes: HgA1C Control, Retinal Eye Exam, Statin Therapy, Kidney Function
- Cardiovascular: BP Control, Statin Therapy, Medication Adherence
- Assessments/Screenings: Fall Risk; Depression Screening, Intervention, Re-assessment; Tobacco Screening and Cessation
- COA Assessments: Advanced Care Planning, Functional Status, Pain, and Medication Review
